Welcome!
If you would like to schedule an appointment, please email me or call me (info below). We can have a free, 15-minute consultation to answer any general questions you may have, to schedule the appointment and get your information to add you to the patient portal. I use a secure, HIPAA-compliant portal that I will invite you to join. There you will find an electronic intake form, consent forms, links to my online store and to your Telemedicine appointment via Zoom, or directions to the clinic.
If you would like to bill your insurance, please call them prior to your appointment and ask the questions on the insurance form below to learn your individual benefits. This will help to avoid surprise charges to your account.
